Join the Practice
Register as a new patient
To register with the practice please complete a registration form and health questionnaire; both forms are available online via the links below. Make sure you complete all the details relevant to you or we will not be able to register you.
After completing the online forms, allow two working days then attend the GP surgery with the following to complete your registration:
- One form of photographic identification, ideally a driving licence or passport
- Your repeat medication slip from your previous practice
Under the new GP contract patients register with the Practice and not with an individual doctor, however you may ask to see a particular doctor or specify a male or female doctor when you book an appointment.

In order for your registration to be processed you must complete both the New Patient Registration and the New Patient Questionnaire forms below. You must also ensure you have the correct documentation described above. If these are not available, your registration will be delayed.
Temporary registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP surgery for 14 days. Please contact the surgery if you need to register as a temporary resident.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with the practice.
To register as a temporary patient simply contact the practice by telephone. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
